I have a 1990 Chevy sierra 2500 4×4 truck that always ran very good until one day driving home it started to run pretty rough. Since then it runs good when I first start it up but once the engine warms up it runs pretty rough and gets horrible fuel mileage.

Bad fuel economy seems to be the key here. If the engine is running that rich, it will cause it to run poorly. You may have a bad fuel pressure regulator. When they go bad, they leak large amounts of gas into the engine causing it to run bad and have terrible fuel mileage.
Or you could possibly have a stuck open fuel injector.
